Receive report from off-going nurse, introduce self to patient, perform safety checks during initial rounding.
Assess patients, check vital signs, check labs, notify MD for any change of patient’s condition.
Perform NIHSS assessment and report any changes to MD.
Monitor cardiac rhythms, notify MD for any changes, give PRN medications, check new orders.
Perform nursing tasks and provide quality care in accordance with hospital policies and procedures.
Administer medications and treatments as directed and as appropriate for patient's medical condition in collaboration with multi-disciplinary team.
Prepare patients for MRI, CT, X-ray when ordered; prepare patient for any kind of surgical procedures and give report to receiving nurse.
Perform re-assessment and document in patient’s chart.
Re-evaluate PRN pain medications and document.
Educate patient and family with their medical condition and reinforce compliance of treatment for better outcome.
Delegate task to patient care assistant according to their scope of practice.

Monitor for any sudden changes with patients' overall health and reporting it to primary care provider, and taking actions per MD orders and hospital policies.

Admit new patient and create an individualized treatment plan for patient. Facilitate groups and provide structural goal for adolescent population. Administer medications and monitor side effects of medication such as EPS. Evaluate vital signs, labs and report to any deviations to MD. Assess patient for suicidal ideation, homicidal ideation, hallucinations, delusions and document it. Provide safe environment for patients to prevent self- injurious behavior. Provide therapeutic communication to patient and evaluate their response. Verify information gathered from patient to their parents or legal guardian for authenticity. Report any suspected abuse to child protection services. Audit paper works from previous shift.

Observe patients ' behavior, listen to their concerns, and chart their condition. Help with admitting and discharging patients. Monitor patients ' vital signs and report changes from baseline. Maintain safe environment for patients.

Assist patients with bathing, oral hygiene, grooming, feeding and elimination.
