> Evaluate patients in the home setting. Review medical history and referral documents , completes assessment to determine physical and psychosocial functioning. Evaluates need for durable medical devices and provides recommendation

> Developing treatment plans: Customize plan of care that outlines patient's goals and expected outcomes

>Implementing treatments/ interventions: Utilise therapeutic exercises, therapeutic activities, functional training , manual therapy, gait training etc to assist patient improve their mobility in the home, reduce pain and prevent injury and hospitalization

> Patient Education: Teach patient, patient's family, caregivers on home program about recovery process and recommend durable medical equipment

> Monitoring Progress: Documents progress and modifying plan of care . Reports to primary care physician or referring physician on outcomes of plan of care

> Supervising Assistants: Supervises PTA and home health aides as appropriate

> Maintaining Patient Record: Keeps track of patient records and goals

> Coordination with Team Members: Communicates with care team regarding patient needs, concerns and outcomes of care

> Completes patient evaluation and developing as well as implementing plan of care for patients in acute care . Patients with neurological, orthopedic, respiratory, cardiac and general medical conditions . Recorded patient response to therapies and made adjustments in treatment plan ,monitored progress

> Coordinated with team members such as nurses, aides, respiratory therapist, occupational therapist, hospitalists, specialists involved with patient

> Communicates and participates with care team meetings and family conference

> Established plan of care , measurable goals for patient for restoration of function

> Provided patient and family / caregiver education and guidance for restoration of function

> Supervised PTA and managed about 6-8 patients a day
