As a case manager for people with psychiatric illnessess, I provide clients with case management and outreach services. Assist clients and their families learn and understand treatment options and making informed decisions, as well as develop and implement goals, assisting, helping and supporting clients in developing symptoms management skills, healthy living skills, linkage of wellbeing activities and resources as well as assisting clients in the properly and effectively ultilizing clinical resources and other social supports, including transportation to medical and other important appointments to promote and encourage, physical, mental and emotional well-being.

As a Mobile Crisis Clinician I was able to assist and help individuals and families experiencing psychiatric crisis. Many of the clients struggled with co-occurring substance use disorder. I worked closely with an array of professional workers such as first reponders, healthcare workers and other community base agencies. The goal was to stabilize clients in psychiatric situations to either avoid hospitalization and/or incarceration and iniciate immediate community-based treatment.

Performed initial intake with clients, developed goals, action/crisis plans, helped, assisted, referred to valuable resources. Assisted clients in securing affordable housing while monitoring their goals and maintaining client's files and reports as required by state rules and regulations. Developed created/built client rapport by meeting with them regularly through one-on-one sessions.

Provided supervision and coordination over residential and community activities for clients in the chemical dependency program. Supported clients with supervision and training on living skills needed to increase independence. Maintained documentation of medication administration, reviewed treatment plans while attended meetings with program personnel, employers, parents, case managers and other services participation, as well as maintained on-going, cooperative communication with services providers.

Conducted directly observed specimen collection of oral fluid, urine, hair, and/or blood. Checked-in clients and enter new-client data into program management database while building relationships with a variety of customers, such as patients, judges, prosecutors, probation officers, and treatment counselors.
