Recorded food, liquid and bowl movements to nurse

as well as any health changes to nurse and family. Planned out different activities including outings to keep patient active during the day and also accompanied patient to doctor appointments when needed.

• Performed light housekeeping like changing bed sheets, keeping house clean and did patient laundry when needed.
• Ordered, picked up groceries, prepared meals and assisted with feeding when needed.
• Provided companionship for patient by engaging in conversation and taking patient on outings once or twice a week.
• Administered prescribed medications on a timely basis following care plan instructions.
• Assisted with transfers from bed to chair or wheelchair on a daily basis.
• Notified any sudden health changes to nurse in charge.
