  • Develop excellence in case management services which will enrich and enhance the quality of life, dignity and respect of people infected and/or affected by HIV/AIDS.
  • Provides support, advocacy, information, resources, and referrals to clients to ensure linkage to care, acting as a liaison between internal and external clients, visitors, and staff.
  • Engages in patient interactions via telephone and in-person to identify, advocate, and meet client needs to ensure linkage and engagement to HIV care.

  • Retrieves and enters clients’ information on assigned systems (ARIES/EPIC) and confirms and verifies clients’ current information.
  • Completely required documentation of services accurately, in a timely manner, and thoroughly in accordance with the department standards to ensure documentation and reporting complies with all regulations.
  • Advocate for clients and assist them in the procurement of benefits and services. Provide necessary information, referrals for established care plans and advocate for client’s needs.
  • Ensure clients are enrolled in medical care and receive appropriate referrals to needed services as established in case plan.
  • Collaborates with a multidisciplinary team while also interacting with a diverse group of clients, visitors, external and internal contacts.
  • Reviews all client forms and ensures client files are complete and in compliance with contract requirements and SYH regulations.
  • Assists the Program Supervisor with completing monthly reports and maintaining an up-to-date and visible monthly calendar of program activities.
  • Performs general clerical tasks as assigned, which may include but are not limited to, filing, processing paperwork, data entry, scanning/photocopying, agenda creation, minutes, encounter reports, scheduling, and other similar tasks and activities.
