Clinical social worker is part of the clinical medical management team and serves as a Behavioral Health (BH) subject matter expert. Social worker independently coordinates and directs the BH and social work services for the Soldiers assigned or attached to the SRU; serves as the BH

clinical care manager; conducts BH risk and clinical assessments; provides crisis interventions; provides BH consultation services; documents in the Electronic Health Record (EHR); serves as a member of the SRU staff for Soldiers and their families.

Social worker works in an independently licensed clinical capacity; works independently with a minimum level of supervision and uses expert knowledge of general and specific concepts, principles, procedures, and practices relating to BH and the social work profession. Provides targeted assessment and evaluation, including diagnostic and functional status impressions focused on the presenting problem(s) and person-in-environment perspective.

BH Risk and Clinical Assessments:

Conducts BH Risk and Clinical Assessments for the SRU Soldiers. Social worker completes complex and comprehensive psychosocial evaluations and multi-factorial risk assessments of every Soldier using standardized tools and professional expertise to determine risk of adverse events and evidence of BH disorders, mood disorders, PTSD, TBI, etc. Social Worker determines health intervention needs, resource needs, issues related to compliance with medical care plans, medical/physical evaluation board actions, and issues related to transitioning back to duty or into the civilian sector. Social worker determines the BH risk level of each assigned Soldier by entering the risk factors into an automated BH risk assessment tool and estimating the risk level using the tool output, professional skills, and judgment.

Clinical social worker performs ongoing BH risk assessments of Soldiers, to include Soldiers who have significant BH disorders, terminal illness, chronic/debilitating illness, are physically abused or neglected, are financially exploited, are incapable of self-care, and/or are mentally incompetent.

  • Behavioral Health Care Management

Monitors BH treatment of the Soldier to ensure Soldier receives required BH care; advocates for quality BH care and/or other services for the Soldier; refer for BH care, when assessed as needing care; track access to care; ensure treatment dosages outlined in care plan are met; provide support to the SRU NCM regarding BH appointment management; ensures that family members and significant others are educated on BH issues of the Soldier (when authorized by the Soldier); document care management of the Soldier in the EHR, and manage Soldier BH care in accordance with the assessed risk level.

Communicates and coordinates with Department of BH at local MTF and/or other care providers (ASAP, FAP, EFMP, community providers) as necessary for referral, access, monitoring, and management of BH care.

Social worker documents care management activities (referrals, contacts, assessed risk, encounters) into the EHR and any other required platforms.

Clinical Social Worker plays a key role in the implementation of the Comprehensive Recovery Plan (CRP) for the Soldier and his/her Family.

  • Interdisciplinary Team Member Role

Communicates on a regular basis with members of the interdisciplinary team and functions as the BH expert, participating in treatment planning conferences and Soldier care related meetings, and participating in or providing input to Comprehensive Recovery Planning.

Communication is focused on BH clinical care management in conjunction with interdisciplinary treatment planning. Social worker, as the interdisciplinary treatment team BH consultant, instructs the team about BH conditions and diagnoses and how to best address and manage Soldiers who may be dealing with these conditions.

  • Soldier Recovery Unit Behavioral Health Services

Perform risk and clinical assessments and provide BH clinical care management. In order to maintain the requisite skill set required to perform these duties and to utilize performance at the highest level of licensure and expertise, the incumbent will complete all activities required to obtain and maintain privileging at their local MTF.

Provide treatment in the context of supportive counseling, brief-solution focused interventions, and crisis counseling to Soldiers. Bridging services and treatment necessary to address access to care issues may be conducted in coordination with the SRU medical management team and MTF BHSL/DHA leadership. Crisis, brief-solution focused, and supportive counseling involves non-therapeutic and therapeutic intervention with individuals and/or groups to lessen the impact of acute, chronic and/or terminal illness or injury on lives of Soldiers and families and to address issues that could negatively impact a Soldier’s treatment plan, rehabilitation and/or recovery. Conducts ongoing family needs assessments with Soldiers and is available to offer family members education, support, resources, and referral (MTF or community) for services.

  • Program Development and Measurement of Program Effectiveness

Coordinates with SRU leadership and conducts ongoing assessments of the social work role and its effectiveness within the SRU. These assessments translate to program development and evaluation, including modification of existing services, as well as development of new programs and clinical services.

Individually and in conjunction with colleagues, conduct ongoing assessments of the effectiveness of the BH clinical care management functional area, and present proposals for change to leadership. On an ongoing basis, conduct individualized assessment of their own effectiveness and review with supervisor during supervision sessions. These will be performed in terms of change seen by clients, quantity and quality of services provided, organizational metrics, and range of services utilized. These assessments will then be utilized in program development.

  • Oversight and Training

Provide a wide range of training on behavioral health topics to Soldiers, family members/significant others, and SRU cadre/staff as well as individuals at other Commands, OTSG, DoD, and external partner programs. Provide feedback to others on the effectiveness of SRU social work interventions as well as needed program changes and modifications to treatment plans.

Provide Traumatic Event Management or other such support to affected SRU staff and Soldiers following incidents such as completed suicides, serious suicide attempts, and related events.
