Pathway Home Program

  • Led the implementation, development, and growth of Pathway Home, a Critical Time Intervention care transition program and CBC's signature innovative program.
  • Expanded the initial two-team program to a citywide, award-winning care transition program operated by 15 agencies, supporting a wide array of populations, and funded by the New York State Office of Mental Health (NYS OMH), HealthFirst MCO, and NYC Health and Hospital Corporation.
  • Attracted, developed, and retained high-performance team members, empowering them to elevate their level of responsibility, span of control, and performance.
  • Established the Pathway Home Training Institute, which now offers weekly workforce training and bi-monthly learning collaboratives through the Project Echo model to PH staff and behavioral health professionals across NYS.
  • Implemented a single point of access (SPOA) referral system for community and hospital referrals to all 15 PH teams.
  • Created, measured, and communicated program plans, milestones, and metrics, including budgets.
  • Ensured that policies were in place around the documentation and standardization of Pathway Home programs and products.
  • Taught bachelor's and master's level seminars on Critical Time Intervention and Pathway Home at Hunter College, City University of New York.

Safe Option Support Program (SOS)

  • At the request of NYS OMH, I led the development and implementation of Governor Hochul's Safe Option Support Teams that support New Yorkers experiencing homelessness.
  • Ensured that the initial four teams were operationalized, conducting outreach and receiving referrals within three months of the Governor's press conference announcement.
  • Created a program design that would bring the Critical Time Intervention model to chronic street and subway homeless clients, and then transformed it into a comprehensive care model that augments current services while offering new post-placement transitional support.
  • Developed a training curriculum to support SOS staff, and enhance care delivery.
  • Provided leadership in the development of inter-team communication and cohesiveness, sustaining culture, and supporting staff during program growth.
  • Fostered collaborative working relationships with key stakeholders, including city and state health and homeless agencies, the Metropolitan Transit Authority, current outreach, housing providers, and Homeless Services United.
  • Developed an SOS Hub designed to act as a SPOA for referrals, provide quality oversight, and report on the work of the initial four teams.

SAMHSA Programming

  • Managed the implementation of the SAMHSA-funded Citywide Addiction Support Network (CASN), consisting of 22 Community-Based Organizations across three behavioral health Independent Practice Associations and three NYC boroughs.
  • Ensured the timely launch of the network and recruitment of peer-led preventative, treatment, and recovery teams that offer comprehensive SUD care across Manhattan, the Bronx, and Queens.
  • Marketed the CASN network through the development of press releases, fliers, merchandise, and information brochures.
  • Managed the services delivery and the training requirements of two SAMHSA State Opioid Response Grants.

Delivery System Reform Incentive Payments Programming (DISRIP)

  • Led CBC's DSRIP participation and networking.
  • Secured DSRIP funding for the creation of several successful, innovative community programs, including Staten Island CARES (in collaboration with SI Performing Provider System (PPS)), Community Outreach and Engagement Services (in collaboration with Mount Sinai PPS), and Bridge Home (in collaboration with Bronx Partners for Healthy Communities PPS).
  • Developed unique program models to address unmet medical and behavioral health care needs of community residents, and in doing so, successfully reduced reliance on hospital emergency room and readmission rates
