Work as a functioning member of a team to assess psychosocial functioning of clients, made appropriate referrals to community agencies, developed disaster preparedness plans, assisted with placement in facilities, and prepared appropriate paperwork to maintain client files.

Provides individual, group, and family sessions to CCSU patients and their families. Completes patient documentation in a timely manner. Assess and monitor for risk, symptoms of trauma, and indications of abuse, neglect and/or abandonment. Uses appropriate reporting mechanisms.

Provide healthcare, transportation, and other various assistance to eligible Veterans and their families. Participate in staffings to discuss progress on families being assisted, and to ensure the best quality of care.

Assessed patient needs and coordinated appropriate social services for individuals and families. Developed and implemented individualized care plans to address psychosocial issues affecting patients. Collaborated with interdisciplinary teams to ensure holistic patient care and support services integration. Provided counseling and emotional support to patients coping with terminal illness and end-of-life issues.
