Martin Olivera-Nino

· Prepares assessments, progress notes, and other clinical documents in keeping with Agency standards as well as HIPAA regulations;

· Conducts assessments, service plan development, housing plan development, case management, psychosocial rehabilitation, crisis intervention, program transfers, and discharges;

· Ensures consumers assigned to present caseload are within County Authorization;

· Generates appropriate electronic chart notes of all consumer services by the County of Sacramento as well as Agency guidelines;

· Provides group counseling and co-occurring skills groups with clients;

· Provides psychosocial rehabilitation and crisis intervention;

· Conducts individual psychotherapy;

· Participates in internal utilization review as well as Agency quality assurance reviews and willingness to participate in QA subcommittee;

· Actively participate positively during internal staff meetings;

· Carries a client caseload and consistently meets established productivity requirements;

· Assess consumers for homelessness, risk of homelessness, or other identified housing needs.

· Provide necessary linkage to resources to address areas about or potentially affecting the client's ability to obtain or maintain housing.

· Completes and submits high-quality case plans and assessments with minimal supervisory oversight;

· Accurately and comprehensively provide a provisional diagnoses of consumer's symptoms utilizing the DSM V and working with a LPHA member.

· Actively contributes positively during El Hogar-provided clinical supervision;

• Identifies psychosocial needs of residents at time of admission and upon staff or physician referral. Makes referrals to community agencies.
• Develops the Social Services plan of care and updates the plan as changes occur for each resident in assigned areas. Participates in resident care conferences which include written notification to residents and families.
• Completes all social assessments, MDS entries, and care plans within required time frames and actively participates in interdisciplinary team process.
• Serves as an advocate for resident's rights.
• Works in conjunction with other departments toward meeting resident's needs.
• Maintain current references of community resources that may be required at time of admission or discharge.
• Communicates unresolved work-related problems using chain of command, consistently demonstrates effective problem solving skills, and handles confidential information appropriately.
