Transportation Coordinator for the non-profit Modus located in Tulsa,OK.

My job duties included processing ride requests, enrolling new clients into the data systems. Coordinating drivers and
ensuring every ride is completed. I communicate effectively with other non-profit agencies that Modus
is partnered with and assist any needs or concerns they may have. I input mileage data entry for the
agency. I also work every Saturday managing the Modus Delivers program. The Modus delivers program is
partnered with Catholic Charities of Eastern Oklahoma and LIFE Senior Services to provide fresh groceries
for elderly clients who are without transportation and food insecure.

• I worked alone in managing a pantry for the Nutrition Program. I ordered items, provided emergency food assistance to clients, managed a rural walmart card program, all data entry for the Dietitian and nutrition department. I handled 104 clients and case staffed clients in crisis. I documented every interaction with clients with a date and time stamp in the data base used at Tulsa Cares. I answered all calls for the nutrition department and scheduled appointments. I also delivered groceries to clients with no transportation. I helped with 2 farmers markets a month.   Provided HIV, HCV, STI point of care test results to Tulsa Cares clients, while adhering to HIPAA and confidentiality requirements.
  Offered/Provided counseling to Tulsa Cares clients who tested positive, before making a warm hand off to a Tulsa Cares case manager.
  Counseled and educated Tulsa Cares clients who tested negative on risk reduction & prevention, offered safer sex practice materials, follow up testing, etc.
  Provided assistance with the referrals process to Tulsa Cares clients.
