Performed nurse case management duties for individuals served on the Big Sky Waiver (BSW) program, the Montana Medicaid waiver program designed to help elderly and disabled Montana residents live at home or in community-based settings, rather than an institution.

Provided Case Management/Discharge Planning for the community’s 36-bed critical access, acute care hospital. Worked closely with patients and their families, as well as community partners, assisting in placement of individuals needing to move to a higher level of care or finding more assistance in the home to maintain independence.
