• Provide weekly wraparound case management services to young adults in the residential program who live onsite in Crossroad’s affordable housing, as well as young adults participating in the job training program

-Host a UTCSW student for 16-24 hours a week

-Guide student through the process of becoming a professional social worker, by providing critical on-site coaching, mentoring, and guidance

-Support student in integrating social work theory, skills, knowledge and values that they have learned in the classroom into a real-world setting

-Provide one hour supervision to student every week

• Work with families experiencing homelessness to ensure available community resources are provided
• Support families in crisis and conduct analysis of needs to coordinate activities and resources to ensure a successful transition to permanent housing
• Maintain landlord relationships through ongoing communications, resolution of client issues, and ongoing inspection to ensure properties are taken care of, and prompt customer service
• Assist families in the Section 8 application process to ensure vouchers are picked up in a timely manner

• Provide holistic and comprehensive services to all clients including intake assessments, goal setting, long-term action plan development, progress monitoring, budgeting, advocacy, and referrals
• Input accurate and complete data for all contacts with clients into Salesforce database
• Connect families with needed and available community resources
• Follow-up with clients and agencies as appropriate to document use/success of referral

• Engage clients in their own futures planning process, provide them with developmentally appropriate, non-stigmatizing, culturally competent, trauma-informed services and supports
• Establish and maintain collaboration with external supports and agencies
• Participate in daily meetings with team and weekly meetings with med provider
• Document all visits with high level of detail and accuracy within a timely manner

• Provide direct assistance with work and education/technical training
• Help people obtain personalized, understandable, and accurate information about their Social Security, Medicaid, and other government entitlements
• Systematically visit employers, who are selected based on the job seeker’s preferences, to learn about their business needs and hiring preferences
• Integrate employment and education goals with treatment, coordinating with additional mental health supports weekly

• Develop individualized service plans with measurable goals
• Provide wrap-around service to ensure needs are being met
• Assist with linkage to resources by completing referrals and coordinating with appropriate persons in the community
• Provide education to consumers and their family regarding psychiatric diagnosis and treatment options
• Provide leadership and resources to fellow teammates
• Attend IEP meetings and coordinate with school staff
