In my role with Better Health Group Management Service Organization (MSO), I provided healthcare practices with chronic case management support for Medicare Advantage provider affiliates. I lead a team of nine nurses providing CCM to the top 5% of the most ill patients with chronic conditions. Developed plans of care module templates, onboarding processes, provided and developed educational tools, hiring new employees as well as assisted the team with developing an EHR case management platform. The daily goal was to prevent avoidable hospital admissions and encourage better health, as well as reduced health plan cost. Current month metrics provided to Leadership weekly and monthly.

My role as CM was to work with avolunteer driven, non-profit organization committed to providing top quality medical and dental services free-of-charge to eligible residents of Pasco County, Florida.
