Provide nursing care to patient at their homes and other facilities including

-assessment of physical , social , emotional needs and environmental conditions

-development of comprehensive personalized plan of care to insure recovery from illness and optimal functioning in home environment with maximum independence and safety

-collaboration with physicians and multidisciplinary health care team

-direct patients care : ileostomies , colostomies , nephrostomies , foleys and suprapubic catheters , wound care to different types of wounds including pressure ulcers, vascular and diabetic ulcers, surgical wounds, NPWT and etc .

Worked as charge nurse at outpatient dialysis centers

Provided direct care to patients undergoing dialysis

performed assessment of patient, management and care of dialysis access ( CVC, AVG, AVF ), administered medications , vaccinations, fluid administrations.

Supervised performance of technicians, developed and implemented nursing care plans, provided education to patients and their families , initiated multidisciplinary care conferences in regards to patients physical , emotional and social needs.

Provided direct care to short term, long term and sub acute patients.

Engaged in assessment of patient , medical documentations, computer interfacing , as well as administered oral and IV medications, common procedure including Foley and suprapubic catheters, PICC lines, ostomies , wound care .
