• Provide screenings, assessments, and intake evaluations for new clients, ensuring thorough clinical evaluation and immediate treatment planning.
• Deliver individual and group counseling focusing on substance use recovery, life skills development, and problem-solving in a multi-disciplinary team.
• Lead treatment plan reviews in collaboration with case managers and therapists, ensuring comprehensive care that supports mental health, housing stability, and educational goals for clients.
• Act as client advocate, liaising with legal, social service, and housing organizations to ensure the successful transition of clients from treatment to independent living.
• Provide crisis intervention and de-escalation techniques, helping clients in distress and ensuring their safety and well-being within the program.

• Perform drug screening (UA) and monitor medication adherence as part of client compliance and progress evaluation.
• Maintain timely, accurate, and comprehensive documentation of client progress, treatment plans, and discharge summaries in EHR in accordance with HIPAA guidelines.
• Coordinate referrals to external social services, education programs, and mental health professionals for holistic treatment of clients.
• Participate in staff meetings, case conferences, and shift exchanges, providing input on client care and aligning treatment protocols with program needs.
• Transport clients to appointments and community events, ensuring they have access to vital resources and support networks for recovery.
• Deliver educational workshops and skills-building sessions aimed at fostering sustainable sobriety, resilience, and self-sufficiency.
