Managing Health and Wellness:

• Provide strategic leadership for resident care in the community by managing, directing, and monitoring the nursing team as applicable by State/Province law, to promote the health and wellness of the resident population.

• Provide clinical care through the direct application of the nursing process; perform and document resident assessments, progress notes, evaluates changes in care needs, complete Individualized Service Plans, provide or delegate hands-on clinical care as indicated by the plan of care and evaluate resident outcomes.

• Collaborates with physicians, pharmacists, and other clinical providers to coordinate care and services for the residents within the community.

• Partner with Neighborhood Coordinators to promote an integrated and collaborative approach to wellness operations and resident care.

• Collaborate with Sales partners to determine community capability and assess potential residents’ appropriateness for movein.

• Provide strategic leadership for resident care in the community by managing, directing, and monitoring the medication care managers, as applicable by State/Province law, to promote the health and wellness of the resident population.

• Provide oversight of the community medication management program to promote resident safety in the medication process including onboarding, training, and performance reviews.

• Provide clinical training and education, as needed, to nurses, medication care managers, care managers and others who provide resident care. Perform skill evaluation to assure clinical capability of care team members.

• Track, trend and report clinical quality data to identify risk.

• Actively participate as a member of the community Quality Assurance and Performance Improvement committee.

• Lead clinical quality and process improvement initiatives within the community to mitigate risk and improve resident care outcomes

• Responsible for infection control programming, including delegation of infection control preventionist, as per state requirements.

• Assure compliance with all Federal, State/Provincial, board of nursing and other applicable regulations.

• Communicate with community healthcare professionals, including physicians, discharge planners, nurses, and state agencies on the care needs of all residents in the community
• Coordinate incidental medical, dental, vision, hearing, and podiatry care for residents
• Perform nursing duties as directed and in accordance with the appropriate scope of the Nurse Practice Act, including administering medications and performing treatments as ordered by a physician
• Initiate assessment resident pre-admission assessment prior to each resident's move-in date and complete routine and as-needed reassessments in accordance with company policy and state regulations
• Complete assessments to potential residents and families and obtain medical records and other requirements as outlined in Title 22 for admission to the community
• Input resident's information and medications on Point Click Care upon admission(move in)
• Ensures service care plans updated in a timely manner and reflects on the plan of care
• Completes documentation in a timely manner, organizes information in a logical manner with entries containing the required information and details
• Administers medications for residents
• Coordinates and supervises client care with collaboration with care managers
• Supports the Memory Care Director regarding clinical concerns and behavioral management for residents with Alzheimer's and Dementia

• Communicate with community healthcare professionals, including physicians, discharge planners, nurses, and state agencies on the care needs of all residents in the community

• Renders nursing care to community members within an assigned unit
• Observes and reports symptoms and conditions of residents
• Takes and records temperatures, respiration and pulse
• Administers medications and IV's prescribed by physician and records reactions
• Sets up treatment trays, prepares instruments and other equipment, accompanies and assists physician with treatments
• Assists in administering highly specialized therapy with complicated equipment
• Maintains records reflecting community member's condition, medication, and treatments
• Assists in maintaining a physical, social and psychological environment which will be conducive to the best interest and welfare of residents
• Supervises the work of Nurses, CNAs, RNAs and all supportive Nursing Staff
• Maintains a working knowledge of resident's condition at all times
• Reviews reports from previous shifts to determine changes in status of residents and assigns duties to nursing personnel
• Responsible for notifying Director of Nursing Services (DNS) and/or Administrator of changes in patient condition or other job-related problems
• Utilized critical thinking skills to prioritize nursing interventions based on patients'' acuity levels and individual needs.
• Conducted thorough patient assessments to identify changes in condition, promptly notifying physicians and initiating appropriate interventions when necessary.

• Provide supervision to Caregivers and Med Techs
• Interpret existing policies and procedures to Caregivers/Med Techs, residents, families and visitors
• Keep abreast of current federal and state regulations, as they pertain to Nursing
• Assist with the administrative authority, responsibility and accountability of directing the IL Department
• Submit employee accident/incident reports to the NM and Director of Wellness within twenty-four (24) hours after their occurrence
• Delegate administrative authority, responsibility and accountability to personnel as deemed necessary to perform their assigned duties and responsibilities
• Supervise, evaluate and counsel all direct resident care for your assigned nursing unit and initiate corrective action as necessary
• Ensure that disciplinary action is administered fairly and consistently, and without regard to race, color, creed, national origin, age, sex, religion, handicap or marital status
• Participate in the employee evaluation process by providing information on the performance of Caregivers and Med Techs
• Maintain a good working relationship with other department supervisors and managers
• Ensure that work areas are maintained in a clean, safe, comfortable and attractive manner assuring necessary equipment and supplies are maintained and operable
• Ensure that Caregivers and Med Techs follow established infection control procedures at all times
• Ensure that Caregivers and Med Techs follow established procedures governing the use of labels and GHS/MSDSs
• Ensure that only trained and authorized personnel operate departmental equipment and that they do so in a safe manner
• Ensure that department equipment is cleaned and properly stored at the end of the shift(s)
• Report and participate in the investigation of all allegations of resident abuse, neglect and/or misappropriation of resident property
• Meet with each resident on a regular basis to provide health and wellness-checks and answer any health-related questions
• Review all resident information and complete an assessment of the resident prior to move-in, when changes in the resident's condition occur, and as required by residence policy and state regulation
• Review the resident's medical report prior to move-in to ensure that the facility can provide for the resident's individual needs and regulatory requirements have been fulfilled
• Order medications and oversee the resident's medication schedule for residents receiving medication administration services
• Ensure proper documentation for medication assistance/administration
• Assist the Nurse Manager to develop and maintain an up-to-date individual service plan for each resident
• Maintain ongoing communication with the resident, resident's family and physician regarding the resident's medical needs
• Coordinate additional health services the resident may need
