  • Reviews daily medical records for record integrity, accuracy, and completeness.
  • Maintains confidentiality of protected health information according to HIPAA standards.
  • Identifies and reviews duplicate patient charts in our EPIC EHR system. Investigates and reviews charts in order to process and complete duplicate chart merges in EPIC. This also includes our PACE patients, and I have complete communication with the PACE OP Team to determine which MRN to keep.
  • Participated in various duplicate chart validation projects and the completion of chart merges for new clinic acquisitions, including but not limited to ChapCare, Paramount, HSK, and Seville, etc.
  • Works and manages the HL7 Interface Error Queue that pertains to patient's labs and radiology results from external vendors (UMI/LAPCORP/QUEST/CHLA)
  • Works and manages the RME Claims Error Workqueue and External ID Bundle Workqueue.
  • Working knowledge with Master Patient Index.
  • Processes the release of information requests as needed (including, but not limited to, subpoenas in the past) for the use and disclosures of PHI in accordance with healthcare requirements and standards.
  • Participates in mammogram imaging reproduction as needed.
  • Participates in specialty catch-up days that pertain to filing pertinent patient documents from external/specialist providers into the EPIC system using OnBase to close the loop on referrals.
  • Follows up with specialty offices for missing reports or incorrect data on a document, specifically with UMI reports.
  • Handles incoming calls when in the office and directs callers to the appropriate department or employee. Also, takes on calls for the Quality Team as needed.
  • Participates in quality reviews and audits, including the chart selection for patients and the submission of documents to appropriate health plans. This also includes our PACE participants.
  • Identified trends affecting productivity or quality improvement initiatives.
  • Conducts and participates in time studies to help establish times to complete tasks within our department.
  • Assists in training employees regarding Health Information Management processes, including Duplicate Chart Merges, Identity Management Interfaces, Overlays, and RME Claim IDs.
  • Participates in meetings relating to changes impacting the organization's HIM operations, and serves as a liaison between UMI and our EPIC Interface team to streamline the process between the two systems.
