Most impactful event was the creation of the Daily Safety Huddle, a 15" forum to discuss safety concerns within the last and next 24-hours thereby significantly reduced email communications, creation of visual controls and indicators to trigger what to report, constructed a reporting guide, and built a system to proactively anticipate and prevent harm events.
