• Verify patient medication history with patient or family.
• Audit Pharmacy Technicians to help obtain the Best Medication History Possible.
• Document proper medication names, dosage, and frequency.
• Contact the patient's pharmacy to get the patient's medication list when the patient or family can not recall or is unavailable.
• Clarify information with the patient or family when the medication history provided had conflicting information. For example: Duplicate/double medication therapy with different milligrams/dosing each.
• Call inpatient rehab/nursing facilities following HIPPA guidelines to request medication records on patients' daily medications.
• Help the medication history team in multiple areas when needed.
• Clarify with the patient or family when the medication list provided by the patient has different strengths or dosing compared to our external source history.
• Prioritize medication history requested by Doctors, Nurses, or Pharmacists.
• Contact the Pharmacist when a medication that the patient no longer takes or does not take at home has been ordered.
