• Collaborate with internal partners including but not limited to social workers, health educators, pharmacy staff, and ECM/community support team members.
• As assigned Justice Involved (JI) Liaison, ensure that incarcerated individuals receive necessary pre-release services to support transition into the community with connection to Enhanced Care Management services
• Review and authorize referral requests for Enhanced Care Management and Community Supports services
• Ensure confidentiality of utilization review, quality assurance information, and individual beneficiary identification
• Maintain thorough knowledge of DHCS CalAIM initiative

• Collaborate with Health Services Administrator to ensure that medical and mental health services are appropriately provided at three separate correctional facilities
