Duties included adverse drug reaction reporting, therapeutic interchanges, extensive parenteral nutrition dosing & monitoring, pharmacokinetic dosing and follow up, medication dosing (enoxaparin, warfarin, DOAC’s, Vancomycin including AUC, gentamicin, Vancomcyin, Gentamicin, Tobramycin and Amikacin, antimicrobial stewardship and IV to PO recommendations). Strong background in clinical pharmacy, includes code participation as well as handling STAT med needs such as norepinephrine, diltiazem among others.
