- Maintain a daily caseload of 9 to 15 inpatient acute rehab patients and complete full discharge planning needs, to include overcoming any impediments to establishing a safe discharge plan.
- Annually managed 330 patients per calendar year and established safe discharge plans for each patient to include any needed follow up services such as home health, DME, waiver aide services, nursing facility services, LTC placement and follow up appointments. Successfully had over 86% of my patients discharge home with home health services.
- Assess clients' needs, develop discharge plans and monitor progress towards goals. Attend and facilitate the weekly team conference which occurs to remain in compliance with CMS guidelines and review any impediments from a medical, physical or home environment standpoint. These weekly conferences include case management, attending physician, nursing supervisor, PT, OT and ST. Post team conference report developed for each patient and updates provided to patients and their family members to review discharge date, any impediments, recommendations for discharge needs and scheduling family training sessions/conferences.
- Adhere to ethical principles and standards to protect clients' confidential information. Work with each patient to make sure I am keeping the correct members of their family up to date on progress and discharge plans.
- Optimize communication between patients and the healthcare staff working with them daily through mini huddles twice a week, weekly team conferences and rounding on each of my patients daily during my working hours.
- Review treatment plans and patient's length of stay to help manage the correct needed length of stay to obtain set goals by patients and their therapists. Able to manage the length of stay of my patients in the calendar year to 2.4 days under the expected length of stay all while patients met needed goals and successfully discharged to home.
- Participate in professional development activities related to case management best practices. Active member of stroke committee for which our facility obtained both JCAHO certification and Encompass certification for stroke center of excellence.
- Utilize multiple computer systems to include ACE IT for charting, Careport for home health, SNF and long term care facility referrals, Parachute for ordering DME needs and individual insurance agency website to submit clinical documentation.
- Serve as preceptor of our department and have successfully precepted 2 full time case managers and 2 PRN case managers within the last 18 months.
- Attend daily med team meetings to ensure patients are progressing medically.
