Maintain a daily caseload of 9 to 15 inpatient acute rehab patients and complete full discharge planning needs, to include overcoming any impediments to establishing a safe discharge plan.
Annually managed 330 patients per calendar year and established safe discharge plans for each patient to include any needed follow up services such as home health, DME, waiver aide services, nursing facility services, LTC placement and follow up appointments. Successfully had over 86% of my patients discharge home with home health services.
Assess clients' needs, develop discharge plans and monitor progress towards goals. Attend and facilitate the weekly team conference which occurs to remain in compliance with CMS guidelines and review any impediments from a medical, physical or home environment standpoint. These weekly conferences include case management, attending physician, nursing supervisor, PT, OT and ST. Post team conference report developed for each patient and updates provided to patients and their family members to review discharge date, any impediments, recommendations for discharge needs and scheduling family training sessions/conferences.
Adhere to ethical principles and standards to protect clients' confidential information. Work with each patient to make sure I am keeping the correct members of their family up to date on progress and discharge plans.
