- Collaborate with interdisciplinary team of healthcare and social service providers to address patients' needs during and outside treatment.
- Manage patient care through closely monitoring blood pressure and recording patients' medical vital signs.
- Educate and counsel patients and families through treatment process, as well as explaining post-treatment home care and self-management of dietary needs and fluid restrictions.
- Care for 30-60 patients (dependent on day of week) with quality-driven support.
- Minimize staff and patient infection risk by cleaning and disinfecting equipment and maintaining proper wear of PPE.
- Deliver updates in patient status and promptly notify charge nurse, APRN and Nephrologist of any issues that occur during treatment via Cureatr.
- Monitor and track patient vitals in Snappy to keep team informed of changing conditions and identify any adverse reactions.
- Gather lab specimens, ordered testing and prepared for laboratory transport.
- Offer immediate assistance in emergency and routine situations to evaluate needs and deliver care.
- Ensure dialysis machines are set up correctly by stringing machines, priming dialyzer and bloodlines, setting machine to Dr's individualized orders.
- Verify treatment orders relevant to dialysis treatment for each individual patient.
- Initiate, monitor, oversee and conclude dialysis treatment.
- Cannulate permanent fistulas, grafts and catheter accesses, clean and dress permanent and temporary catheters.
- Administer medications including intravenous solutions.
- Train new department staff and monitor performance.
- Routine water checks ensuring parameters are within policy-specified ranges.
- Internally disinfect machines and water loop weekly.
- Stock supplies throughout day as needed, organize stockroom and put away incoming stock pallets.
