For over 9 years, I led a multi-disciplinary care plan team in long-term care to complete MDS assessments/care plans accurately and timely and led care plan meetings to ensure residents received quality care. I achieved Signature MDS Champion and served as point person for MDS coordinators and Rehab Directors in our region.
I currently lead a skilled nursing facility’s infection control program, including surveillance of nosocomial infections to mitigate the spread of infection throughout the facility.
