Summary

For over 9 years, I led a multi-disciplinary care plan team in long-term care to complete MDS assessments/care plans accurately and timely and led care plan meetings to ensure residents received quality care. I achieved Signature MDS Champion and served as point person for MDS coordinators and Rehab Directors in our region.

I currently lead a skilled nursing facility’s infection control program, including surveillance of nosocomial infections to mitigate the spread of infection throughout the facility.   • Use information from MDS and CAAs along with other pertinent information to develop Plan of Care and update at least quarterly and with each Comprehensive Assessment to assure changes are included.
