Clinical Document Specialist-Inpatient Coordinator

  • Gather and process inpatient medical and surgical documents
  • Verify patient history
  • Check for current medical insurance coverage that is currently active
  • Add the ICD 10 code for the reason the patient is admitted as an inpatient or for an inpatient surgical procedure
  • Assign CPT (medical billing code) for the patient’s procedure if one is to be completed
  • Monitor document systems for security issues
  • Contact the facility to inquire if the patient was admitted for advanced admission medical/surgical procedures
  • After admission is confirmed, begin calling the facility the following day to attempt to verify the patient’s discharge date
  • Once admission and discharge are verified, the case can be closed with the code notification requirement met
  • Check for duplicates when verifying the initial request for authorization from the facility that was received
  • In some cases, a phone call to the facility may be necessary to verify that the member did not leave one facility and was admitted to another the same day
  • Collaborate with intake and customer service when the facility calls them with questions
  • Assist intake and customer service by providing them with the reason for a duplicate cancellation or cancellation and rebuilding the case because it was initially built incorrectly by intake
  • Collaborate with the facility when a case must be denied
  • Troubleshoot with the facility to verify if the case can be updated so it can be approved or canceled and rebuilt in order to submit approval for the patient and facility.
