Regularly collaborate with practitioners by recording and sharing physical therapy outcomes and recommending modifications to treatment plans.
Outpatient Clinic
Evaluate and assess patients to establish measurable goals and promote restoration of functional abilities.
Maintain organized documentation and patient files.
Identify and properly document patient goals, evaluate progress and plan reevaluation.
Customize conventional physical therapy techniques to meet specific needs of individual patients.
Create individualized home exercise programs for clients to improve and maintain progress.
Early Intervention
Formulated, implemented and documented treatment plans for children 0-3 years of age. Children who were typically developing, but delayed in their gross motor skills due to medical conditions ,low tone and hypermobility etc.
School based; ages 3-21
Evaluated, formulated, implemented and documented treatment plans for special needs children of various ages with various diagnosis. This included but is not limited to IEP, HEP, equipment and orthotic clinic.
Outpatient Therapy; Pediatric -Geriatric
Evaluated, formulated, implemented and documented treatment plans for individuals of various ages with various diagnosis.
Subacute-Rehabilitation Center; Adult-Geriatric
Screened, evaluated, treated and completed documentation for the residents of CLHC-RC . Diagnosis of individuals included amputations, lupus, CVA, MS, cardiac pathologies, balance defects, joint replacements etc.
Outpatient orthopedic setting; Pediatric - Geriatric
Screened, evaluated, treated and completed documentation of patients. Diagnosis were all orthopedic in nature they included TKR, RTC Repair, meniscal tears, spondylolisthesis, muscle strains, tendon sprains, ORIF, Scleroderma.
School based pediatric physical therapy:3-5 years of age
Home health care setting; Adult-Geriatrics
Treated patients with diagnosis that included CVA, Parkinson’s, OA, balance deficits, strength deficits
