• Collaborate with Case Manager and Lead Case Manager to support the reunification program and ensure timely case management services.
• Enroll youth in the internal intake system and maintain accurate electronic case files.
• Manage and secure inventory of youth’s personal belongings and ensure compliance with DHS and ORR documentation requirements.
• Facilitate two weekly telephonic contacts with youth’s families, primary caregivers, and sponsors to promote ongoing communication.
• Coordinate and facilitate legal service presentations and attorney-client communications as needed.
• Conduct follow-up service calls 30 days post-reunification to ensure continued support for discharged youth.
• Maintain a high level of organization and confidentiality while managing multiple priorities in a fast-paced environment.
• Demonstrate proficiency in communication and software applications such as Microsoft Office and UAC portal.
