• ADLs . Assist patients with ADL's; clean personal spaces; patient cleaning and hygiene; bed baths and shampoo; change bedding and gown; follow nutrition plans; feeding as directed (prepare food or g-tube).
• Vitals . Take and record vital signs; photograph and immediately report physical changes and sores to nurse or doctor.
• Mobility . Help patient walk, or if bed-ridden administer range of motion exercises to avoid atrophy.
• Wounds and Rx . Administer wound dressings and medications under direction of nurse and physician. Photo records of dressings to nurse.
• Bedsores . Perform and teach care-givers how to position and reposition patient to avoid bedsores; discussed mattress changes with nurse and team.
• Fall prevention . Recommended grips and fall-prevention strategy to nurse, team, and family.
• Kindness . Interacted and provided emotional support to patient and family members. Reported continuing problems to nurse and team.

On Saturdays only, one particular patient

• ADLs . Assist Amy with daily living activities, such as toileting, transfer sofa-wheelchair-toilet.
• Bedsores . Apply ointment (Stage 1)
• Medications . Keep track of and provide prescribed Rx.
• Emotional support . Kindness, comfort, and conversation with Amy and her husband Richard.

• Physical therapy . Hands-on exercise to restore range of motion for permanent patients and surgical recovery. Walk patients to help mobility.
• Bedsores . Position and reposition bed-ridden patients.
• Vital signs. Full work-up, blood pressure, pulse, temp, etc.
• Transfers . Bed-wheelchair-toilet.
• Fall prevention . Lower bed for patients confused or coming out of anesthesia.

• ADLs . Total-care patients: grooming, hygiene, all bodily functions, g-tube feeding.
• Bedsores . Position and reposition patients every 2 hours.
• Vital signs . Full panel every day.
