Job Summary

Provide skilled nursing care within a multidisciplinary environment using the HOPE model to ensure safe and effective patient care. Manage individualized, goal-directed nursing care through the use of the nursing process and the principles of primary nursing, in accordance with departmental and hospital policies and procedures.

Duties and Responsibilities:

• Provide direct nursing care to patients on a shift basis in a defined clinical setting

• Prepare plan of care for each patient using nursing process, patient’s needs assessment, planning, implementation and evaluation using the HOPE model and best practices option.

• Demonstrates the skills and judgment necessary to implement medical/nursing plan of care, nursing interventions, and procedures, as necessary for the care of the patient.

• Evaluates the identified problems, care provided, and patient’s responses that are reassessed, in order to meet patient and family needs to ensure overall quality of care delivered

• Complete and document a full nursing assessment, the patient’s nursing/medical history, drug history, nutritional history, immunization history and pain management as required.

Duties and Responsibilities

• Triages each patient and determines priority of care based on physical, psychological and social needs as well as factors influencing patient flow through the emergency care system

• Performs focused assessment of chief complaints of each patient in a timely manner.

• Recognizes need for assistance in treating patents and calls for appropriate help.

• Assists physician with scheduled procedure or examination of the patient in emergency room while providing for patient comfort, safety and privacy.

• Notifies other departments when their services are needed (Ex. lab, x-ray, pharmacy).

• Observes and carries out all doctor’s orders which may include: applying dressings, administering and/or dispensing medications.

• Prepares and maintains a sterile work area from skin preparation through procedure to dressing application.

• May assume duties of either circulating nurse or scrub nurse in the minor operating /procedure room.

• Manages contaminated instruments and supplies during procedures to prevent spread of infection and reduce risk of exposure to bloodborne pathogens.

• Prepares the patient for various examinations like X-ray, Ultrasound, CT scan etc.

• Performs some examinations of the patient like ECG and blood sugar monitoring, VBG,blood extractions.

• Administer medications and treatments as prescribed by the physician.

• Performs Intravenous Insertion, Nasogatric tube Insertion, and Foley catheter insertion

• Performs special minor procedures like posterior molding, casting, eye and ear irrigation

• Assist in various procedures like Intubation, Chest Tube Insertion, wound suturing, thoracentesis, paracentesis and other minor procedures performs at the minor OR.

• Manipulate equipment/apparatus like cardiac monitor, administer oxygen therapy.

• Facilitates referrals to higher level institution

• Facilitates admission, transfer to the designated area of confinement, and discharge of patients.

• Act as wardman (the one who transport patient) if deemed necessary.

• Encoded every patient to the Electronic Health Management system in the unit.
