Certus September 2005 - Present Clinical Intake Coordinator Certus Healthcare has 5 local facilities and I work asa Clinical Intake Coordinator/Liaison for those facilities. The day to day workload entails receiving referrals and process the referral from start to finish. This process includes working with the hospitals discharge planning-social services department to accepted patient to one of our facilities. The process also includes working with the prospective patients insurance company to obtain insurance authorization. The process also includes speaking with the potential patient and or their family or friends who are involved in their care to ensure a seamless transition from hospital to their next level of care. I also work with physicians offices and Passport services admit those who need long term care into along term care facility without requiring a hospital admission. My original position within the company starting in 2005 when we were Zandex Healthcare, I was an LPN on the 11p -7a shift. After approximately 1 year on the 11p-7ashift, I moved to the 7am-3pm shift. I spent approximately 5 years as a floor nurse providing routine nursing care. I then became the facilities wound care nurse for approximately 1 year. Our company was opening an independent skilled facility and I transferred to the new facility to be their nurse aide coordinator and housekeeping supervisor. My duties were to hire, schedule and provide ongoing training to the facilities state tested nurse aides and to the facilities housekeepers. I also worked one day per week asan LPN. I was in the nurse aide coordinator position from 2012 until 2018. My company was sold and the new owners business model did not include my position. Continuing Healthcare Solutions took over in 2018 and I transitioned to the Admissions Director. Since 2018, in the role of Admissions Director, I have also worked in the community as a Marketer. Our company was sold to Certus Healthcare in 2021 and I have remained working in the Admissions Department in some capacity. My current role is Clinical Intake Coordinator/Liaison.

I was a basket manufacturing facility, making handmade quality baskets. After approximately 4 years of employment and the company having mass layoffs during those 4 years, I did return to school to obtain LPN license so when and if I was to be laid off, I would be able to find employment. I continued with my employment after I had my LPN license until my position was cut due to a mass layoff.
