- November 2018 – November 2021: Community Health Worker / Quality Improvement Specialist with the Department of Family and Community Medicine at the Robert B. Green Campus with UT Health System. I assisted the clinic with patient care by ensuring the health and care of patients is met at every visit with lab and testing orders, medication refills, attestation forms ICD codes are addressed with the provider prior to the patient coming in to the clinic for an appointment. Followed up with patient post clinical visit to assist in scheduling mammogram and colonoscopy orders. Reviewed the patients medication adherence to make sure patients were taking their medications as prescribed and initiate refills for those in need to stay in compliance. Collected and prepared Attestation forms from insurance portals and distributed to the Family Health Clinic and General Medicine Clinic for open care gap closure and HCC review.
