- I worked at the geriatric facility, and my role included admission and discharge planning, looking for placement for patients either in nursing homes, assisted living, or sending clients back home to their loved ones after treatments.
- I also did follow-ups after they were sent home to ensure that they were having a smooth transition. I conducted psychosocial assessments, held group meetings, completed census tasks, communicated with family members about any changes in clients' conditions, called DHR because we had clients who were under their care, and we had to update them about changes.
- Attended weekly meetings with doctors and nurses to discuss discharge planning, changes in the client's conditions, or any updates from family members. Lastly, I did a lot of documentation, as that is a way for doctors and others in the circle to know about any changes with clients.
