  • working with IDD Individuals not on Innovations waiver as well as members with Mental Health Substance Use diagnosis
  • providing intervention and coordination of care to eligible Vaya members to ensure these individuals receive appropriate assessment, and care

  • As discussed in my interview, I have a student in my classroom this year who has been diagnosed with Severe Elipsepsy
  • He has had this condition since he was 4 months old and he has not had grand mal seizures since he was in PK
  • Recently, he began having those seizures again while in the classroom
  • He also has an IEP so I work with the IEP team as well as a care team that was established this year to make sure he has the best level of care possible in the classroom when he has episodes
  • We have determined the best care plan of action since we created this care team
  • We meet on a weekly basis, when necessary, to discuss higher levels of support and discuss the best care man of action for this student
  • It is important for us as a care team to make sure he is as comfortable as possible in the classroom environment and able to participate with other students and not feel excluded from activities but that he is able to participate as safely as possible and join the other students
  • Although it is my first year with this student in my room, each year we come in contact with students with different disabilities and work with an IEP team or other therapists as mentioned above
  • I have students in my class with IEPs for Speech, Occupation Therapy, and some students with both
  • As a teacher, we are always working with a team even though that team varies depending on what we are working with
  • Each child is different
  • Daily communication with parents, school staff, school administration, and other school faculty
  • Having meetings with the school counselor and other team members regarding students who need a 504 Plan because they do not qualify for IEP services but can greatly benefit in the classroom by having a 504 Plan.
