Accomplishment

• Acts as a liaison and consistently communicates with families, professionals, guardians, and referring agencies to provide best outcomes for the clients. Ensures that these communications are recorded in a timely manner and participates in weekly narrative communication.
• Acts as good steward of Damar and/ or client finances.
• Attends mandatory staff meetings and other meetings as assigned.
• Represent clients in court Reviews and IEP/Case Conferences by providing accurate information regarding the client's treatment progress and areas of need.
• Complete Independent Living skills for the clients.
• Represents clients at doctor's appointments and exchange information effectively and accurately.
• Plan and lead program meetings.
• Leads and directs the implementation of IPP by the providing guidance and training to employees for assigned clients.
• Write and update monthly IPP to ensure they are ready by the 6th of every month.

• Ensure there is referral source communication weekly.
• Serve as an active member of an interdisciplinary treatment team by attending and participating in monthly treatment reviews and weekly meetings.
• Participating in psychiatric reviews for each assigned clients providing direct feedback to the psychiatrist during appointments or written feedback through nursing department.
• Complete and followup on BDDS application and assist with client transition and discharge.
• Facilitates Program Training for unit/group homes staff including Independent Living skill training.
• Ensure services design and delivery provides each client with active treatment.
• Completes follow-up recommendations for services, equipment, or programs.
• Collaborate with Residential Manager to ensure assigned location remain within budget parameters.

Accomplishment

• Effectively responds to and manages crisis situations, which may include client physical aggression, utilizing prescribed verbal intervention or physical management techniques.
• Assist in maintaining a safe and healthy environment, including performing a variety of tasks related to housekeeping including cleaning, washing soiled clothing, changing bed linen, folding and sorting clean laundry.
• Supervised individuals with their Activities of Daily Life
• Assisted with day-by-day operations of the residence accordance with regulatory requirements, agency policy and quality program standards
• Consistently implements behavior management plans to reduce client's maladaptive behaviors, promote appropriate social behaviors and maximize human potential if each client served.
• Completes all required documentation daily, in a professional manner - must be objective, accurate, and thorough to provide consistent client treatment.
• Complete incident reports, including reporting any suspected exploitation, abuse, or neglect of a client.
• Assist in teaching students to acquire basic academic, behavioral, communication, social-functioning, self-help, daily living and social skills.
• Work with clients, professional and other Para-professional staff, client's families and the community in a courteous and respectful manner to provide proper habilitation.
• Assisted residents with personal living and daily care activities in their units, group homes and community and provided direct supervision to the individuals
