• Supervises and performs direct patient care including but not limited to taking and monitoring vital signs, initiation and termination of dialysis, documentation, physical assessment of patients and patient treatments, taking and transcription of physician's orders, drawing and documentation of ordered laboratory work, training patient and/or care partner to perform dialysis at home, and perform home evaluations as required 30% 2.
• Seeks to facilitate growth of home program and leverages tools and resources for effective program management, supervises day-to-day work of PCTs, is a resource for new RNs and other teammates.
• Assesses patient and family readiness to learn and provides appropriate information on renal disease, prescribed dialysis treatment, and medical regimen and its impact on the patient's state of health and well-being while creating a training plan utilizing DaVita education programs. Independent and proactive collaboration with IDT for new and ongoing patient education. Participates in the development and implementation of the patient's short- and long-term plan of care and monthly progress note.
• Analyzes patient assessments, laboratory data and, when concerns are noted, brings issues to the attention of the physician and the IDT. leads PCC discussions and leverages AI model output to assess trends and make recommendations to care team.
• Communicates all emergencies, patient life-threatening complications and related incidents to supervisor, Medical Director, Regional Operations Director, Facility or Group Facility Administrator. Notes signs of trauma, patients' general physical and mental condition, reviews dialysis orders, documents observations in patient medical records
• Provides peritoneal dialysis therapy within the scope of practice and according to the plan of care; recommends a dialysis plan for ultra-filtration; complies with all safety measures, monitors patient vital signs applicable to ESKD patients, administers medications per physician instruction if allowed by state agencies, documents activity appropriately.

• Provides quality direct patient care and renal replacement therapies, in a professional, considerate and respectful manner while interacting and communicating with hospital partners, gives pre/post treatment report through appropriate reporting process to hospital primary RN/Staff
• Provides initiation and discontinuation of the prescribed renal replacement therapy, including but not limited to formulating a plan per physician orders, verifying safety measures are in place and complied with, administering medications or other appropriate treatments, and documenting all activity appropriately on the approved flow sheet or hospital EMR
• Documents patients' physical assessment, vital signs, treatment reviews and observations in patient medical records and brings issues to the attention of the physician and primary nurse after analyzing patient assessments
