• Responsible for monitoring in-basket messages in EMR and addressing them timely and routing appropriately.
• Identifies patient care problems and communicates them to physician on-site.
• Maintains up to date and accurate documentation of nursing care, in EMR.
• Rooms patients for provider visits, takes vital signs and weights. Documents patient concerns and questions.
• Administers medications and immunizations as ordered.
• Supports providers by doing chart prep/chart review for upcoming appointments by updating health maintenance measures and tracking lab and radiology orders in the EMR.

All other duties as given while practicing 100% within scope of practice.

• Assists the practitioner with examinations, procedures, and other duties as assigned.

• Administer chargeable/VFA/VFC vaccines to children & adults.

• Exposure screening, specimen collection and submission to lab, quarantines/isolation of pets, outreach, and related follow-up.
• Educate community on public health concerns, issues and most specifically family planning.
• Work in conjunction with schools that offer “School based health” to run immunization clinics for their employees and students.
• Keep records & count inventory every month to ensure proper amount of vaccines. (Chargeable and vaccines received from the state.)
• Abide by any and all regulations and laws put forth regarding NYS law.

• Work directly one-on-one with medical providers (MD, DO, PA, NP’s) as the practice facilitator 3-4 times a week in a clinic setting.
• Facilitating referrals ordered by medical provider.
• Scheduling nurse and provider appointments.
• Keep records or perform other clerical duties in doctors’ offices or clinics.
• Collect samples, such as blood, urine, sputum from patients, and perform routine laboratory tests on samples and/or send out to lab as needed.
• Blood draws, suture/staple removing, wound care/dressing.
• Prepare patients for examinations, tests, or treatments and explain procedures.
• Evaluate nursing intervention outcomes, conferring with other healthcare team members as necessary.
• Set up equipment and prepare medical treatment rooms.
• Give prescribed medications, make and change MARS as needed throughout the month.

• Data collect during “Open Wellness Hours” & follow treatment guidelines for appropriate treatment. Forward to RN or medical provider for further evaluation as necessary for treatment.

• Administer prescribed medications, noting times and amounts on patients’ charts
• Observe patients, charting and reporting changes in patients’’ conditions, such as adverse reactions to medication or treatment, and taking any necessary action.
• Provide basic patient care or treatments, such as taking temperatures or blood pressures, dressing wounds, treating bedsores, giving enemas or douches, rubbing with alcohol, massaging, or performing catheterizations.
