• Provide skilled nursing care to adult and geriatric patients in the home setting, including wound care, medication administration, IV therapy, ostomy care, Foley catheter maintenance, and diabetic management.
• Conduct comprehensive patient assessments and develop individualized care plans in coordination with physicians, case managers, and interdisciplinary teams.
• Perform Start of Care (SOC), Resumption, Recertification, Follow-Up, and Discharge visits with accurate documentation per Medicare and agency guidelines.
• Educate patients and caregivers on disease management, medication compliance, nutrition, and fall prevention to promote independence and safety at home.
• Monitor changes in patient condition, communicate critical findings to providers, and adjust care plans as needed to prevent rehospitalization.
• Ensure compliance with infection control standards, HIPAA regulations, and agency policies.
• Maintain detailed charting in EMR systems (e.g., Kinnser, WellSky) to support timely billing and quality assurance reviews.
