· Staff floor nurse providing care for inpatient acutely ill patients with a ratio of 1: 3 on a 14-bed intermediate care unit.

· Patient population includes GI bleeds, Afib with RVR, SVT, hypoxic respiratory failure, heart failure exacerbation, Asthma exacerbation, BIPAP/CPAP/ HFNC requirements, COPD exacerbation, DKA, ESRD complications, sepsis and other infectious complications.

· Duties include managing patients who decompensate rapidly and need thorough assessments and observations. Maintaining professionalism and teamwork on the unit and communicating with interdisciplinary team members.

· Supervision patient care technicians /unlicensed assistive personnel.

· Cardiac rhythm interpretation.

· Workplace violence prevention.

· Recognizing acutely decompensating patients.

· Managing continuous drips to include diltiazem, amiodarone, nicardipine, insulin etc.

INTERNSHIPS

• Utilizes Nursing Care Plan for individualized nursing care. Proper drug administration technique and monitoring therapeutic and untoward reactions of medications. Proper Blood administration and Transfusion and monitoring patients before, during and after. Taking EKG, familiar with basic EKG patterns and arrhythmia. Interpret basic and common EKG patterns and any abnormalities. Taking blood samples for laboratory analysis(ABG, I-Stat) Accurate monitoring, measurement of Intake and output then computing fluid balance on end of the shift. Accurate documentation of blood/fluid loss, especially post operative clients. Monitoring and documentation of vital signs, neurologic status including pain. Interpretation and recognizing significance of results of laboratory tests on blood gases, electrolytes, blood chemistries, and other tests. Assisting in the removal of chest tube and assist in minor surgical procedures performed in the unit such as: tracheostomy, lumbar tap,liver biopsy, peritoneal dialysis, chest tube insertion, thyroid needle biopsy. Initiation of emergency resuscitative measures such as CPR. Performing tracheostomy care and wound dressing. Implements pro-patient’s safety: fall precaution, seizure precaution. Providing individualized patient/family education with the client and his/ her family.
• Cases Handled. Cardiac Arrhythmia(SVT,AF, A-fibrillation, PACs, ventricular fibrillation, atrial flutter), Dilated Cardiomyopathy, Mitral Stenosis, Cardiomegaly, Congestive Heart Failure, Hypovolemic Shock, Hyperlipidemia, Lung Cancer, Hypertension, , Pericarditis, endocarditis, pericarditis, myocarditis, Miliary/ Pulmonary Tuberculosis, Bronchial Asthma, Post CABG surgery, Aneurysm, Coronary Obstructive Pulmonary Disease, Post Kidney Transplant, Chronic Kidney Disease, Diabetes Mellitus, Community Acquired and Hospital Acquired Pneumonia, Coronary Artery Disease, Cardiogenic Shock, Septic Shock, Cardiomyopathy, Peripheral Artery Disease, Deep Venous Thrombosis, Pulmonary Edema.

· Perform patient assessment/reassessments on a regular basis in accordance with established policy.

· Assesses, develops, implements and evaluates a comprehensive plan of care through a multidisciplinary care team process in conjunction with the patient/family in internal and external settings.

· Associate remains flexible to changing systems, is expected to demonstrate quality and effectiveness in work habits and treat staff, physicians, patients and families with consideration and respect.

· Provides care to as many as 5 medical/surgical patients at a time on a 31-bed floor.

· Quick to respond in emergent situations and consistently uses clinical reasoning to provide quality care.

· Types of surgeries/medical conditions include urology, orthopedic, gynecology, infectious disease, pulmonary, trauma, oncology, geriatric, thoracic, nephrology, psychiatric, etc.

· Experienced multiple rapid responses and a few code blues. Consistently monitoring and recognizes clinical deterioration.

· Trained in pressure injury identification and staging as well as chest tubes care.

· Electronic health record used is MedConnect PowerChart.

· Comfortable with the use of the Alaris pump, as well as PCA pumps, JP drains, chest tubes, external fixation devices, IV medication administration including insulin drips, heparin drips, etc.
