I provide an array of services to residents at the Union Grove, Green Bay, Chippewa Falls Veterans Housing and Recovery Programs to include Individual and or group psychotherapy services to adult. Conduct assessments of client needs using clinical interview, objective diagnostic tools, and collateral contacts. Develop treatment plans collaboratively with clients and provide agreed upon and necessary therapeutic services in keeping with strength-based, evidence-based best practice standards. Maintain detailed clinical records and make recommendations concerning case disposition. Complete agency related phone calls, email, and paperwork in a professional and timely manner. Adhere to high ethical, professional, and legal standards such as WI State Statutes, Health Insurance Portability and Accountability Act, Counsel on Accreditation, American Psychological Association, American Counseling Association, American Association for Marriage and Family Therapy, and National Association of Social Workers. Advocated on the behalf of Veterans and dependents with claims for State and Federal benefits that they may be entitled to. I acted as a Power of Attorney to Veterans or their dependents when representing their claims for specific needs to obtain State or Federal benefits.
I coordinated the care of the homeless/chronically homeless Veterans in the HUD-VASH program. Screened referrals, confirmed eligibility, and managed admission to the HUD-VASH program. I participated in crisis intervention, multidisciplinary team meetings, and family counseling. Provided intensive case management, treatment and supportive services to Veterans. Assisted Veterans with improvement of their overall health, social connectedness, and function at their highest level within the community. Case Manager for the Waukesha Alcohol Treatment Court. I monitored and provided case management services to individuals with three or more OWI offenses. Case management services include intake evaluation through discharge from the program by successful completion of the program or by the inability to follow guidelines of the Waukesha Alcohol Treatment program. I provided structure, accountability and support for a successful recovery. Made referrals treatment and other community based support services. I met face to face with clients in the office or at their home while maintaining regular ongoing contact with all pertinent client activity and status regarding treatment, attendance at self-help groups, employment, alcohol/drug tests and SCRAM results, ancillary services and the client’s family life.
