  • Dear Homemaker-Home Health Aide Applicant:
  • New Jersey law (N.J.S.A.45:11-24.3 et seq.) requires that every person who applies to become certified as a homemaker-home health aide MUST undergo a Criminal History Record Background Check. An important part of this process is the recording of each applicant's fingerprints. Your fingerprints will be used to check the criminal history records of the New Jersey State Police and the Federal Bureau of Investigations (FBI).
  • The Division of Consumer Affairs uses the Live-Scan Fingerprinting Service provided by IDEMIA, a private company under contract with the State. As an applicant, you are required to contact IDEMIA to schedule a time and place to have your fingerprints recorded. The quickest and easiest way to schedule your appointment is via the web at https://uenroll.identogo.com. Web scheduling is available24 hours per day, seven days per week. Applicants without web access should call IDEMIA at the company’s toll-free telephone number,1-877-503-5981 (Monday through Friday,8:00 a.m. to5:00 p.m., and Saturday,8:00 a.m. to12 noon). Spanish-speaking operators are also available upon request.
  • When scheduling an appointment with IDEMIA, you will be required to provide the following information: Service Code –2F1K6F (this code is specific to your license type) and Contributor’s Case Number –2609969 (this number is specific to you as an applicant).
  • You must bring valid identification with you to your appointment. It must be a government issued identification with a photograph and it must include your name, address (home or employer) and date of birth. A passport or driver’s license with a photograph will meet this requirement.
  • Please be advised that the criminal history record background check must be current at the time of licensure. Therefore, the criminal history record background check must not be older than six months at the time you are to be licensed. If the application process extends for more than six months from the date the criminal history record background check was conducted, the Division will be required to conduct a new F.B.I. background check.
  • Additional information about the criminal history background process is available at http://www.njconsumeraffairs.gov/ocp/Pages/chr.aspx. This site will also explain how to complete or challenge the accuracy of information obtained from a state or federal record check. The Criminal History Review Unit is available by telephone at609-826-7184 to address any of your questions or concerns. This letter is intended only for your use. Do not photocopy and distribute this letter because it contains information applicable to you. Please note that if you reside outside of the State of New Jersey, you should contact the Homemaker Home Health Aide Unit at973-504-6546 to receive special instructions on how to complete this process.
  • Pursuant to N.J.S.A.45:11-24.5, the Department of Law and Public Safety will assume the cost of the criminal history record check, so long as you appear for the fingerprinting, with the required documentation, as scheduled. Appointments must be canceled by noon on the business day prior to your scheduled time (you must cancel by Saturday at noon for a Monday appointment). If you fail to cancel your scheduled appointment, you will not be rescheduled until you pay IDEMIA a fee for failing to cancel in a timely manner.
