• Telephone triage
• Perform nurse visits such as: blood pressure follow-up, INR management, immunizations, and newborn weight checks
• Inform patients of medical test results
• Assist with prescription refills
• Assist with minor office procedures
• Update and maintain accurate documentation in electronic medical record
• Provide patients and families with health education regarding a variety of health conditions.

• Collaborate with operational leaders, IS partners, clinical staff, and providers to facilitate new processes related to the electronic medical record
• Provide nursing perspective for changes in the electronic medical record as part of organization-wide initiatives
• Project management for small scale projects and request-level work

• Provide nursing care for patients with actual and potential health care problems using the nursing process
• Plan and direct the nursing care for individual patients, groups of patients, and their families
• Work alongside physicians, therapists, and social workers to provider holistic interdisciplinary care
