• Interpret prescriptions from healthcare providers to properly dispense medications and health products.
• Vaccinate patients to provide immunity against influenza, pneumonia and other diseases.
• Recommend OTC devices or medication options to help mitigate individual symptoms.
• Supervise dispensation of controlled substances using careful oversight and audit procedures.
• Manage pharmaceutical supply procurement and management processes.
• Evaluate patient histories to assess medication compliance and spot issues such as doctor shopping or excessive usage.
• Save patients money and increased pharmacy profit margins with therapeutically equivalent and more cost-effective generic drugs.
• Check prescriptions for appropriate dosage, drug interactions, allergies and contra-indications before dispensing medications.
• Provide specialized services to help patients manage conditions such as diabetes, asthma, smoking cessation and high blood pressure.
• Effectively resolve insurance rejections and other billing issues.
• Maintain state and federal records of controlled substances.
• Comply with all federal laws, company policies and procedures and state regulations regarding intern, technician and assistant training and licensure.

• Participate in drug use initiatives and strategies to promote cost-effective prescribing in different clinical areas

- Acne/Rosacea (topical products), Asthma/COPD (inhalers), Cardiovascular (fibrates/statins), Endocrine (oral diabetic agents, thyroid agents), HIV/Hepatitis B (antivirals) etc.

• Perform patient chart reviews and communicate with prescribers to implement conversion to preferred agents different methods: therapeutic conversions, brand to generic conversions, dose consolidation, and biologic to biosimilar conversion
• Pharmacy and Therapeutics Committee Participation: Update and present bi-monthly updates on non-formulary conversion chart; Review and present new drug monographs (KedRAB in November 2019)
• Continually monitor internal database and generate reports to identify new opportunities to decrease use of non-preferred therapies
• Collaborate with pharmacy benefits, systems integration, infusion, Outpatient, Supply Chain teams to streamline drug use initiatives and to identify/resolve any gaps in process
• Communicate with commercial, Medicare Part D, Medicaid, and Health Exchange plan patients to discuss change in therapy
• Publish and update pharmacy department's monthly newsletter "Pharmacy Matters", which highlights current and upcoming drug use initiatives
• Precept pharmacy school students on Advanced practice rotations

• Perform daily clinical chart reviews and medication therapy management on assigned patients to identify drug related problems and to contain drug costs according to clinic practice guidelines, inpatient formulary and clinical protocols: renal dosing, antibiotic review, IV to PO conversions, drug level/laboratory value monitoring, home medication reconciliation
• Interpret prescription orders, dispensed medications and counseled patients on potential drug interactions.
• Evaluate appropriateness of drug therapy based on patient-specific factors and drug information.
• Exhibit proficiency in IV and chemotherapy compounding.
• Work as preceptor/mentor for students completing introductory and advanced pharmacy practicums.
• Supervise dispensation of controlled substances using careful oversight and audit procedures.
• Assist patients in minimizing medical expenses by recommending generic alternative prescription medications.
• Work with automated dispensing systems and other pharmacy automation systems as required by department.
• Verify safety and efficacy of medication use by reporting adverse drug events (ADEs), medication errors and other quality issues in alignment with regulatory policies and procedures
• Review patient medication profiles and medical records to screen for appropriate drug selection, dosing, contraindications, side effects and therapeutic outcomes.
• Dispense IV and PO medications and communicate with prescribers regarding therapeutic incompatibilities.
• Participate in development of drug utilization evaluations, data collection reports, and other quality assurance activities as required to ensure appropriate and safe drug utilization
• Serve as active member of the Pharmacy and Therapeutics (P&T) Committee, presenting drug monographs and education pharmacy and nursing staff on changes to hospital wide formulary
