Duties and responsibilities include but not limited to:

• Evaluates patients referred for physical therapy in accordance with regulating bodies; utilizes specialized testing procedures and skills; incorporates data from patient/family/caregiver interview and assesses the patient's strength, ROM, coordination, sensation, cognition, mobility skills, and rehabilitation potential
• Administers skilled physical therapy treatment and monitors patient response
• Plans for patient discharge including home assessment, patient and family instruction, equipment recommendations, home program recommendations, and recommendations for community support
• Collaborate with others to coordinate care, resolve concerns/complaints, and address the needs of patients, families, staff in a timely manner
• Serves as clinical instructor to PT and PTA students
• Trains and mentors new hires

Lexington of Orland Park provides Skilled Nursing and Subacute Rehabilitation

Duties and responsibilities include but not limited to:

• Evaluates and treats residents, under orders from a physician, through the use of a variety of therapeutic procedures designed to restore loss of muscle strength, coordination and body alignment, to relieve pain and to improve movement and function
• Participates as a member of the interdisciplinary team providing support and information within the area of Physical Therapy
• Instruct Assistants in treatment procedures to be utilized and provide close supervision while such staff members are working with residents

Duties and responsibilities include but not limited to:

• Meets the patient's goals and needs and provides quality care by assessing and interpreting evaluations and test results, determining physical therapy treatment plans in consultation with physicians or by prescription
• Restores patient's function, alleviates pain, and prevents disabilities by planning and administering medically prescribed physical therapy
• Completes discharge planning by consulting with physicians, nurses, social workers, and other health care workers, contributing to patient care conferences

• Duties and responsibilities include but not limited to: - Provides and directs Physical Therapy services to patients including assessment, treatment, program planning and implementation
• Functions under physicians' orders and adheres to applicable principles and practices of physical therapy, Innovative Senior Care policies and procedures, and state regulations

Duties and responsibilities include but not limited to:

• Performs thorough evaluations according to protocol, which are relevant to patient diagnosis, and current scientific and clinical knowledge
• Plans, prepares and carries out individually designed programs of physical treatment to maintain, improve or restore physical functioning, alleviate pain and prevent physical dysfunction in patients
