• Assess client needs and resources, develops and/or implement individual service and treatment plans. Identify professional service needs and make the appropriate referrals. File Annual Assessment updates;
• Provides case management and rehabilitation services that assist and support clients in all areas of daily living. Address and provide support for clients daily medical health concerns in both individual and group settings;
• Provides educational and support services to families in individual and group settings, so that family members may better assist clients and meet the client’s needs;
• Helps coordinate with other work team services members to ensure proper implementation of services including crisis intervention, discharge planning, rehabilitation, and assessment. Provides incident reports, and comply with state and federal regulations regarding health and safety;
• Complete clinical records and other required documentation in accordance to agency requirements. If needed, provide client transportation
services.
