Provided symptom management for patients with various medical conditions including heart disease, COPD, cancer, ALS, dementia and other diagnosis considered terminal. Provided self-directed care with astute judgment skills and a high level of personal accountability. Triaged and responded to emergency calls in a timely manner and administered medications and treatments as ordered. Provided education for caregivers on all aspects of patient care.

Provided care to people with terminal diagnoses in their homes. Care included assessing symptoms and working collaboratively with their attending physicians and hospice medical director in providing appropriate interventions. Cared for patients with terminal CA, endstage heart disease, end stage COPD and end stage dementia. Interventions included IV and subcutaneous pain medications, IV dobutemine therapy, maintaing VAD including PICCS, portacaths. Provided documentation and maintained up to date patient infomation in EMR.

Admission/Clinical Nurse Liason:  Evaluated and assessed patients for hospice eligibility with a thorough assessment of the disease process, past medical history, treatments and medications. Prepared hospital patients for safe discharge home .  Involved collaboration with hospital physicians and case managers as well as hospice team. Initiated the admission process by educating and informing patient and families of hospice services.

RN Case Manager: Assess and manage patients care in their home, skilled nursing facilities, and hospitals. Develop plan of care and explain hospice services and philosophy to patients and their families.Collaborate with interdisciplinary team to provide physical, emotional and spiritual care to patients and their families.
