Participate with Family Court Hearings
Conduct home visits in the home as well as school and daycare settings.
Do referrals for families such as individual mental health services, family counseling,
parenting classes, substance abuse counseling, CASAC, and GABI.
Conduct assessments with family that include Domestic Violence, Mental Health, Sexual
Questionnaire, and Genogram.
Explain to families regarding FTR 3 phases. Phase 1 – Initial, Phase 2 – Baseline,
Phase 3 – Stabilization
Inform families regarding HIPPA and 2921 forms before signing.
Review Safety planning and risk assessment
Work with ACS regarding the next steps with the family as well as schedule home
visits
Engages the family in setting goals, establishing priorities, and implementing
service/treatment plan activities; tracks progress towards desired outcomes; and initiates
solutions to eliminate barriers to goal achievement.Documenting all case activity with families and community providers using the NYS
Connections system and NYC PROMIS system.
Conducts case contact visits in the home and counseling sessions as mandated and based on
need.
Coordinating and maintaining contact with workers at the Administration for Children’s

Complete all required documentation and data entry within deadlines, including
assessments, progress notes, treatment plans, and closing summaries.
Observe and monitor the safety of the environment, both physically, emotionally, and

Adhere to the fidelity of the FTR model: Work within the model structure
regarding case contacts, interventions, supervision, documentation, and length of
treatment.
Complete all required documentation, both the SBC model and ACS.

Monitor and document child contacts in a family for risk and safety as described by the
Administration for Children Services (ACS) standards.
