-While operating independently in the field, 's I would travel to patient's location. Sometimes this was at a personal residence, but mostly at hospitals, and LTC facilities to assess patient's need for hospice.
-Patient's were assessed to see if they were hospice appropriate and if so, along with the patient and patient's family and or friends, what the best placement would be for the patient. This would sometimes be at the patient's personal residence, other hospitals, LTC facilities, virtual beds at Houston Methodist, and also Houston Hospice IPU.
-I worked very closely with Case Manager's, Social Workers, Nurse Practitioners, Doctors, patient's and their families, Houston Hospice doctors, and many other entities depending on patient's needs.
-If patient was approved to go to their residence on home hospice, I arranged for all needed supplies such as a hospital bed and mattress to fit the patient's needs, Bedside table, Bedside Commode, Oxygen concentrator, suction machines, and many other supplies and or services needed for the patient's care.
-Once the patient was accepted to Houston Hospice, I would then arrange the patient's transportation to where ever the patient would be going to.
-Communication between Houston Hospice's Intake department was vital to getting the patient to their needed placement. Outcome emails and phone calls were passed along with each patient explaining, in detail, what transpired with the patient's visit and assessment.
-Collected data about patient care problems and prepared reports.
-As the clinical Liaision, I also educated case managers, social workers, nurses, and doctors on the specific criteria for a patient to be accepted to Houston Hospice. This was done via educational seminars and through daily interaction with Hospital's and other facilities.
-The patient and families were educated on what to expect while on services with Houston Hospice. Any and all questions that may have come up during the visit, was answered to the best of my abilities and if I could not answer a certain question, I always had superiors to call.
-Utilized data regarding industry trends to shape solutions and approaches.
-Collected, reviewed and analyzed data from diverse departments for patient and facility needs.
-Built and deployed plans, reports, validation procedures and entry processes for data management needs.
-Resolved conflicts and negotiated mutually beneficial agreements between parties.
-Another part of my role as the clinical liaison, was keeping up with mileage driven to and from the patient and also keeping up with parking fees to be given to my superior for reimbursement.
Developed team communications and information for weekly clinical Liaison meetings and monthly company meetings.
-During my time at Houston Hospice, I was quickly given the opportunity to train new LVN's and RN's .
-I also held training seminars and educational meetings for hospitals, LTC, and mostly with MD's and Case managers so that they could help with providing patient's and their families with appropriate information about what hospice is and what services we provide.
-The reason for my departure from this company was because I was injured outside of work and required a major surgery on my left foot.
