• Responsible for rooming wound care patients for Providers. This process requires critical thinking skills to recognize potential problems, that need to be communicated to treating Provider.
• Work closely with home health nurses in the field to identify changes in wound condition, and promptly notify providers when necessary.
• Work as a team member to collect, communicate and develop care plans to optimize patient's wound healing.
• Educate patients and their caregivers on proper wound care to minimize the risk of infection, while optimizing wound healing.
