• Manage and supervise program personnel in case management and support areas.
• Establish and maintain program delivery models.
• Ensure that regulations and quality assurance indicators are achieved.
• Maintain client records in a professional and confidential manner.
• Participate in the general management of the facility or program.
• Coordinate and facilitates meetings with funders ( OMH and DMH)
• Facilitates High Risk and Group Supervisions.
  Participates in Case Conferences with external sources.
  Collaborates with other VOA Departments (Facilities, Clinics, Central Intake, etc) for program needs.
  Participates/Performs staff evaluations.
  Ensures regulatory/agency compliance with staff completion of trainings.
• Collaborates with Residence Manager to ensure apartments are safe.
  Participates in administrative meetings.
  Participates in other on/off site meetings as scheduled.
  Participate in Client/tenant meetings and works with staff to address and resolve entitlements issues.
• Perform other related duties as required.

• Manage a caseload.
• Complete Assessment Summary and make determination on type of intervention needed for each family.
• Develop individualized service plans to focus on recovery and wellness.
• Maintain service documentation to include client progress notes, observations and interactions about clients and incident reporting when necessary.
• Provide on-call crisis intervention support.
• Participate in individual service coordination activities, including vocational and educational services and support recreational and/or socialization activities.
• Maintain program’s resource directory.
• Conduct outreach to community-based organizations.
• Refer families to appropriate community providers to address family needs.
• Supervises assigned staff.

• Gather information for and/or conduct outreach, intake, assessment, reassessments, and care plans.
• Provide care management to a caseload through the coordination of medical, mental health and substance abuse services.
• Implement activities to facilitate participant engagement.
• Participate in case conferences with health home team including, Case Managers and treatment providers.
• Ensure entitlements, insurance, and benefits are in place.
• Develop service plans and resolve barriers to effective service utilization.
• Responsible for assisting Health Home team members in implementing individualized care plans.
• Monitor client's progress in utilizing services (appointments, treatment, medication, etc.) through telephonic and direct contact.
• Check that clients receive test results and track that client follow up with medical directions.
• Escort clients to/from medical appointments when needed.
• Collaborate on care issues with internal clinicians and the participants assigned Health Home medical provider.
• Document service information in databases and use electronic Clinical record systems to track participant activities.
• Assist in crisis intervention and provide or refer to crisis services.
• Conduct home visits and community work in the field including visiting hospitals and emergency rooms when determined necessary.
• Ensure that clients follow up with aftercare discharge (i.e., fill prescriptions, make appointments.

• Admits clients who are displaced by fire, natural disaster or structural damage to HPD's emergency shelters and hotels by completing and submitting intake forms for further processing.
• Prepares and complete a Bio- psychosocial assessment on all clients assigned by the field Instructor.
• Develops a comprehensive housing plan and review plan on weekly basis to ascertain the actual outcome.
• Advocates for vulnerable clients whose present conditions (Physical, emotional and psychological) is an obstacle to getting permanent housing, by helping client obtain housing vouchers.
• Helps clients to complete and submit eligible housing application.
• Referred clients to housing resources and other relevant agencies including hospitals and other mental health facilities.
• · Records and updates client’s data on weekly basis and provides progress notes on each interaction with client on daily basis using AWARDS
• Ensures that all documents belonging to both the client and the agency are kept confidential.
• Conducts room inspections as well as other wellness checks.
• Ensures that clients comply with all shelter rules and regulations.
• Assessing clients' physical and mental wellness, needs, preferences and abilities, and developing plans to improve.
• Work with clients, family and friend support networks, and health care professionals to put care plans in place.
• Listening to clients' concerns and providing counseling or intervention as required
• Evaluates clients' progress periodically and adjusting as needed.
• Follow up with discharged clients to ensure they are satisfied with services and still in good physical and mental health to transition them into permanent housing.

• Screen those patients whose diagnosis and social data indicate need for social service intervention, according to Social Work practice standards.
• Make psychosocial assessments utilizing all available data, including interviews with patient and family, and conferring with interdisciplinary team members.
• Ensure continuity of care through development, implementation, and validation of discharge / treatment plans, in a timely manner and in collaboration with patient, family and interdisciplinary team.
• Function independently but utilize supervision to identify any problems in expediting discharge / treatment plans.
• Maintain Social Work professional conduct and practice
• Participate in individual service coordination activities, including vocational and educational services/workshops, and support recreational and/or socialization activities.
