The Senior Case Manager has the primary responsibility of assisting their peers with onboarding, ongoing case consultation, and shadowing as well as providing Case Management services to a variety of populations, following applicable County, State and Federal guidelines.
Attend all required in-services and unit meetings as scheduled or review the minutes within one week of the training/meeting.
Assist with the orientation and ongoing training of all assigned professional personnel, including but not limited to providing shadowing opportunities of meetings and follow up paper work.
Assist management to ensure each assigned employee is provided orientation to all items on the orientation document form.
Complete peer internal quality audits with assigned employee during orientation period and as needed. Ensure case managers are provided additional training and understand the audit requirements.
Provide person Centered Services
As requested, interview prospective individuals at intake and review disability status
Conduct interviews, collect and compile assessment data, social histories and/or diagnostic information necessary to establish or update case files.
Obtain consent for the Release of Confidential Information from the person served or their legal representative during the first face-to-face contact or via mail and renew the consent on an annual basis.
To maintain positive, open, and respectful communication with individuals served, families, providers, county representative, staff and other involved persons.
Attend and/or facilitate Support Team meetings. If unable to attend, attempt to reschedule the meeting. If not possible to reschedule, request that a replacement Case Manager attend the meeting for you.
Assist individual served and their support team in defining emotional, physical, social and cultural needs, in and developing an overall plan of intervention.
Solicit and encourage expression of personal values, goals and dreams with every individual served.
To meet the cultural and language needs of persons served.
Act as an advocate to the person served in exercising their citizen rights. Facilitate review of major life decisions with the individual or designated guardian/conservator.
Review guardianship status at least annually, and present necessary information to county or district court, as necessary.
To assist the person served in locating and gaining access to necessary waiver services (if eligible) as well as medical, social, educational or other needed services, regardless of funding source.
Assist person served in determining eligibility for financial assistance and/or social services.
Monitor services provided to the person to evaluate effectiveness in meeting service needs. All individuals served, with the exception of those on AC and EW waivers, must have at least two face to face visits per year. Individuals on AC and EW waivers must have at least one face to face visit annually with twice annually being best practice. Initiate contact with the individuals served or their legal guardian at least quarterly is best practice and encouraged for all individuals served.
To document all activity performed on behalf of the person in the case notes. Ensure that notes outline all follow up activities performed and time is accurately reflected to match billing.
Provide referral to needed supportive services. Conduct tours and complete necessary applications as needed.
As reports or other documents are received, review the document, document the review in applicable case-noting system, and place the document in the individual’s file. If there is any follow up needed or anything unusual noted, document observation in the case progress notes.
E1MN liaison with Dakota County. Committee made up of Dakota County staff, contracted case management (I was only staff that filled this role) and Vocational Rehabilitation Services. Training within Thomas Allen Dakota County Case Manager, support any employment questions and problem solve with co-workers.

Provide direct care and guidance to consumers (adults with disabilities) of various abilities. I am in charge of the vocational area at the Parkway Site, assuring the availability of contract work, building work skills, and job coaching. I also provide staff with adequate training for the vocational area. I also supervise three staff members in my area. Paperwork for consumers: OPRs, behavior support plans, outcomes, program development, and skill building. Paperwork for staff: quarterly reviews, monthly employee developments, problem-solving, and goal setting.

Senior Internship

While completing my Senior Internship at Theresa Living Center, I worked with the residents. There were between 12 and 15 women receiving housing during my time at Theresa Living Center. They came from different backgrounds with different needs. Some women were single, and some had one child ranging from 6 months to 2 years old. I provided supervised case management services and access to resources they needed to achieve self-sufficiency, such as rental assistance, counseling, job training and employment opportunities, and access to information and referrals. I met bi-weekly to track goals and plan toward self-sufficiency. I also helped support program goals: providing a safe, nurturing environment for participants, collaborating with existing agencies to address the specific needs of homeless single women and their children, providing individualized guidance to participants in planning their life goals, and ongoing evaluation of their progress toward the goals, strengthening participants’ basic home management and life skills, including budgets and financial planning, and helping and encouraging participants in their search for affordable, permanent housing.
